 Reservation Guidance |
1. Who must make reservations?
The following four kinds of people, who apply for work, study or live in China and are required to apply for Residents Permits, Temporary Residence Permits, Employment Permits or visa extension, must make reservations:
(1) Foreigners
(2)Chinese citizens (Hong Kong and Macao residents)
(3)Chinese citizens from Taiwan Province
(4)Chinese mainland citizens settling overseas
Chinese who apply to go abroad don’t have to make reservations for the physical evaluation.
2. Either personal users or group users should not submit fake information for online reservations. Once a reservation is made online, our system manager will check the authenticity of the information provided.
3. Please enquire about the service time of physical examination before you make a reservation online.
4. After making an online reservation, you can enquire about the reservation information submitted by yourself, and can change or cancel your reservation.
5. Only registered users can make reservations online.
6. Both the 1st and 3rd buildings, 15 Jinbang Rd, accept reservations on spot and provide the services of physical examination for foreigners and overseas Chinese. Please follow your reservation instructions to go to either Building 1 or Building 3 directly for check-in and examination on the day of examination.
| How to make a reservation online? |
1. Log onto our website www.sithc.com, and click the “Report Inquiry & Reservation” button to enter the log-in page;If you haven’t registered, please register first. Make sure you use the same ID number you will use when visiting our center, otherwise you won’t be able to inquire your physical examinations results online;
2.Input your “NAME” and “PASSWORD”;
3.Click “Online Reservation” in the “RESERVATION” on the left page;
4.First step of making a reservation: choose your expected date of examination;
5.Second step: click the “TIME” with vacancies available;
6.Third step: Fill in the full names of “Applicant & Contact person”;
If the applicant and the contact person are the same one, please input one name, and then input the contact number and the contact address; click “SUBMIT” to finish;
7.On the page that displays reservation information, you can check the information registered, and click “DELETE” to delete faulty information or cancel reservation. Please keep your reservation number, and you can also click and download the “Physical Examination Application Form”, to save you time of filling form when visiting our center. You can also click “Legal Health Administration” on the page, to check examination notices, time, and documents required;
8.Click “EXIT” after you’ve made the reservation.
