【Ways of Reservation】
There’re three ways to make reservation:
1.Phone calls :62688851,business time 8:00-17:00.You can leave a message during non-business time，and we’ll call you back within one business day.
2.Fax:62683088. Please leave your name, phone number, company’s name, number of people who need exam, occupations on the fax. We’ll call you back within one business day.
3.Website:www.sithc.com which only provides regular reservation service, and not enjoys the priorities of the Easy Access.
【Conformation of “Application Form of Health Assessment”】
Applicants who make appointments through the Easy Access should complete "Applicantion Form of Health Assessment ".
- When you make reservation, we will make sure your physical examination date, time, place, number of applicants, occupation and payment. And we’ll notice you the phone number of our doctor in charge, your reservation number, materials for physical examination and ways to collect your results.
- Reservations should be made by the real names. For group reservation, applicants’ name list should be followed by their passports. - After reservation, we’ll issue an "Application Form of Health Assessment ". If there’s no mistake, please let your commissioner general sign on the form and make your official seal, and fax the it to 021-62683088. (One group physical examination appointment needs just make one "Application Form of Health Assessment ")
【Health Examination Application Form】
Applicants need fill up the “Physical Examination Form” in advance.
The form should be with the official seal, and on the day of physical examination applicants should show the form to the staff.

【Certificate-collection】
There are three ways to collect your certificates:
1.By mail. Charge :RMB 25 YUAN.
2.Group authorized collection
Person should show an AUTHORISATION LETTER with sign and pavilion official seal, his ID card/original passport+one photocopy of ID card/original passport to collect the results.
3.Pick up the results in person or authorized person at the Counter No.25 "Inspection and Quarantine" at Expo service center .
